What is Nurse Led Care?

Understanding nurse led care and what to expect from our care services
Nurse led care is care that a registered nurse will take the lead on, and this is what we provide with our healthcare services at home. It is often referred to as complex care, and basically, it means that every client will receive a tailored care plan that is devised by a registered nurse. The trained carer will carry out their work on the back of the plan and the registered nurse will ensure any clinical care is monitored. With Homecare services, it is vital that it is nurse led as this ensure that all care provided is safe and suitable for the client. The care plan provided is always tailored to suit the needs of our client, and the condition they are suffering from. There are no two conditions that are exactly the same. For instance, although motor neurone disease is degenerative, some people can live with the condition will require end of life care within just a year or so of diagnosis, others can live with the condition for 10 or more years. It is important that we provide a care plan that is completely tailored and monitored continuously to ensure that it is line with the progression of the disease.

At Harmony Healthcare, our nurse led care involves an initial assessment to discuss the needs of the service user and devising a care plan that is suitable for their needs, no matter what they may be. Ensuring risk assessments are carried out and that the care plan is constantly monitored to ensure it is supporting the client, and the nature of the changes in their condition. The nurse will also provide hands-on care, together with the carer to ensure that the best possible level of care is offered at all times. They will ensure they communicate regularly with the family to ensure that all needs are taken care of and that the goals of the individual are always kept at the forefront of everyone’s minds. Advice and support will usually be required for the family, and our client to ensure they are living as safely and well as possible. Our nurses will also collaborate with other organisations who are providing care to the client, ensuring that their needs are always known and that everyone is working together towards a collective goal.

What are the clinical service options you provide?
We offer our clinical services to a wide range of people, including paediatric care  for babies and children, as well as for complex conditions, such as Parkinson’s and cerebral palsy  We can also provide palliative care for those that need it. Our nurse-led care includes feeding support and respiratory support.
How do I know what care I need?
You don’t need to worry about trying to work out what care services you need, we will do this for you. After you have contacted us, we will arrange a visit to find out more about your condition, and what we can do to help. We will create a tailored care plan based on your needs. It may be necessary to receive around the clock care, or you may need short term care. We are completely flexible in our approach to the nurse-led care we provide and are happy to adapt it to suit your needs.
How long does it take to set up the care plan?
We are in operation 24 hours a day, 365 days a year, so we are always on hand to deal with your queries, if you need care in an emergency care.
How do you select a carer?
We provide carers that are best suited to your needs, taking into account your interests and the activities you enjoy, as well as your values. We believe this is paramount in ensuring you receive the right level of care, and above all, that it is enjoyable. Our carers often become friends with our clients, which is something we believe to be important to enable us to provide the best possible level of care.
Are the carers properly trained and experienced?
We only provide carers that are passionate about providing care and offering an exceptional level of service. Our carers are trained and experienced to do their job, and they must have experience to join us.

Our carers also receive clinical training to be able to provide any complex care at home. It allows them to be able to deal with clinical interventions and take a hands-on approach to Homecare. The training is delivered in a bespoke way, which allows us to ensure that the carers are trained specifically for the needs of each individual client.

We don’t just hire any carers, we only recruit those that are passionate about delivering care, and of course, that they are empathetic, caring people. It is our priority to ensure that the care we deliver is consistent and that it is always delivered to suit the needs of each individual.
